Five Easy Steps

1. Preview: $100 Rebate, Research Abstract, FHCi Consent to Research, Research Registration Parameters, HPx Program fees

2. Become an FHCi Patron: $45 donation, includes consent to research. Check email for link to Patron Portal

3. Set up your Patron Portal, sign PMA agreement, and add famliy members

4. Enter the Practitioner Directory, interview practitioners, and select your HPx Practitioner

5. Register for HPx Programs, complete the Inclusion/Exclusion forms, and schedule an HPx Program initial appointment to commence HPx Program(s)
